What this guide covers

Identify the entity, facilities, products, FTL decisions, and CTEs actually performed. Gather procedures and records before filling the worksheet.

Practical implementation

For each activity, identify the owner, source system, record linkage, data checks, retrieval steps, and escalation route. Maintain revision control and update after meaningful changes.

Limitations and verification

The worksheet is not a complete legal checklist for every entity; confirm specific plan elements in Subpart S.

Build the plan from the real process map

Use the worksheet to identify the required plan elements and link each to an approved procedure, record location or named role. Avoid writing “records are maintained electronically” without naming the system or explaining how records are retrieved. If records are maintained by a service provider, state the retrieval route and the contact responsible for obtaining them.

For the FTL identification procedure, link the product-scope register and explain how changes are reviewed. For TLC assignment, describe the applicable assignment point, owner and record linkage. For the point of contact, name a role and backup process so the plan remains usable after staff turnover. Farms with the relevant covered activities should check the exact farm-map requirements in the regulation.

After drafting, walk through a sample lot and confirm that every statement matches actual practice. The worksheet is not a substitute for the regulation or a prescribed FDA form.
